I , , whose signature appears at the bottom of this page, on my own behalf and on behalf of all persons staying with me, hereby release and hold harmless Veterans Care International (VCI), Las Flores, and Maple Suites, including their respective staff, representatives, and service providers, from any and all responsibility, liability, or claims arising from or related to my participation in sports or other activities at the pool, sea, beach, floors, common areas, accommodations, or any other area of the premises.

I further release and hold harmless Veterans Care International, Las Flores, and Maple Suites from any responsibility or liability for incidents resulting from my own negligence, imprudence, or failure to exercise reasonable caution, including any damage caused to the property, facilities, furniture, equipment, or other belongings of Las Flores, Maple Suites, or Veterans Care International.

I also acknowledge that Veterans Care International, Las Flores, and Maple Suites shall not be held responsible for activities conducted by concessionaires, independent contractors, or licensed third-party companies, nor for incidents resulting from failure to follow posted signs, instructions, or safety regulations, or for incidents related to the abuse or use of alcohol or drugs.

I further acknowledge that Veterans Care International, Las Flores, and Maple Suites are not responsible for money, valuables, jewelry, personal belongings, or documents that have not been properly secured in the safes or other designated storage facilities provided to guests.

As the services are provided in the Dominican Republic, the undersigned agrees that any controversy or dispute arising from or related to the services provided to guests shall be subject to the jurisdiction and competence of the courts of the Dominican Republic, and expressly waives any right to submit such matters to any court or authority outside the Dominican Republic, to the extent permitted by applicable law.

By order of the Central Bank of the Dominican Republic, payments must be made in Dominican pesos based on the official exchange rate.
