3. PROGRAM REQUIREMENTS & CONDUCT STANDARDS

3.1 Participation in Wellness Activities

You agree to:

• Participate in the scheduled wellness activities included in the program
• Arrive on time and be prepared
• Follow instructions from Program staff

3.2 Behavior & Conduct

You agree to:

• Treat all staff, wellness providers, veterans, and hotel personnel with respect
• Avoid aggression, threats, harassment, or disruptive behavior
• Avoid creating unnecessary conflict or disturbances
• Follow all hotel policies and Program guidelines
• Maintain a cooperative and respectful attitude
• Refrain from unsafe or disrespectful actions
• Leave the room intact and on time for the scheduled check-out

Our shared commitment to safety, mutual respect, and program integrity is essential.

Actions that compromise these standards may result in removal from the Program.

If you have concerns or require support to meet these standards, please contact Program staff as soon as possible. Violations will be reviewed and addressed in accordance with Program policies.

3.3 Alcohol & Substance Use

Las Flores Hotel & Wellness Center maintains an alcohol- and substance-free environment during wellness hours.

Intoxication that affects participation, behavior, safety, or the experience of others is not allowed.

3.4 Guests & Unauthorized Visitors

• Guests must be approved by the Program
• Guests must sign the applicable agreement before participating
• Unauthorized visitors are strictly prohibited
• If a guest violates Program or hotel rules, they may be asked to leave the premises
• The veteran may also be subject to removal from the Program as a result of serious or repeated guest violations

3.5 Program Focus

This program is not a party trip. It is designed to support:

• Wellness
• Recovery
• Relaxation
• Stress reduction
• Mental and physical improvement

Anyone participating for purposes unrelated to the Program may be removed from the Program.

3.6 Extra Activities and Free Time Policy

Due to high demand for our services and providers, the Program maintains a structured schedule during business hours.

Participants may enjoy personal activities during free time, provided these activities do not interfere with scheduled Program activities.

About Extra Activities:

• Extra activities must only be planned during free time.
• Please do not schedule external excursions during scheduled wellness activity hours.
• Extra activities must not cause fatigue, intoxication, delays, or inability to participate.
• If an extra activity causes you to miss or skip a scheduled wellness activity, you will be considered in violation of Program requirements.

4. GUEST (PLUS-ONE) OBLIGATIONS

If you bring a guest:

• Your guest must sign the applicable agreement before approval
• Your guest must follow all Program rules
• If your guest violates Program rules, both the veteran and guest may be subject to removal
• Guests must respect hotel rules, staff instructions, and behavior expectations

Guests May Participate In:

• Meals included with the Program
• Catamaran excursion
• Sound therapy
• Free time at the hotel
• Pool, beach, and social time

Guests May NOT Participate In:

• Wellness sessions
• Veteran-specific activities
• Therapeutic sessions

4.1 Transportation Policy — Veteran-Only Transportation

The Program provides transportation for the veteran only, including:

• Airport pickup and drop-off
• Transportation to scheduled wellness activities
• Transportation to included Program events

Guest Transportation Rules

Guests may join Program transportation only if they arrive and depart on the same flight and schedule as the veteran.

If a guest arrives or departs at a different time or on a different day, transportation will not be provided.

Guests may use Uber or other transportation services at their own expense.

5. FINANCIAL RESPONSIBILITY & COSTS

Although veterans do not pay the full cost of their participation, the Program invests significantly in each participant. All participants must comply with Program requirements to maintain their sponsored participation.

5.1 Daily Sponsorship Cost — 2026-2027

The following costs are covered by sponsors to allow eligible veterans to attend the wellness tour:

Failure to participate in scheduled Program activities may result in loss of sponsored accommodation for that day, as described in Section 6.

5.2 Meals

Breakfast is included every morning.

Depending on the day's schedule, either lunch or dinner will also be included and served at the hotel.

The other meal may be at a different restaurant and will be at the participant's own expense.

5.3 Credit Card Authorization

NO AMERICAN EXPRESS — AMEX IS NOT ACCEPTED IN THE DOMINICAN REPUBLIC

By signing this agreement, you authorize the Program to charge the credit/debit card provided for applicable expenses resulting from:

• Non-participation
• Damages
• Excessive cleaning
• Guest misconduct
• Additional expenses caused by your actions

5.4 Emergency Room (ER) Visits

The Program is not responsible for ER visits or related expenses.

If you choose or need to go to the ER:

• You are responsible for all medical or hospital bills
• You are responsible for transportation
• The Program does not reimburse ER expenses
• The Program is not responsible for delays or consequences resulting from an ER visit

This Program does not include emergency medical coverage.

6. SECURITY HOLD & TOUR FEES

2026 — $200 REFUNDABLE SECURITY HOLD

A $200 USD refundable hold will be placed on your card at check-in as a security deposit for your room.

The $200 USD hold will be released 72 hours after check-out, once the room has been inspected and provided that there are no applicable charges for damages, excessive cleaning, missing items, or other expenses resulting from the participant's stay.

The $200 USD hold is not a tour fee and is not a charge unless applicable expenses are identified.

2027 — $200 NON-REFUNDABLE TOUR FEE + $100 REFUNDABLE SECURITY HOLD

For 2027 tours:

$200 USD — NON-REFUNDABLE TOUR FEE

This fee is required for participation in the 2027 tour and is non-refundable.

$100 USD — REFUNDABLE SECURITY HOLD

A $100 USD refundable hold will be placed on your card at check-in as a security deposit for your room.

The $100 USD hold will be released 72 hours after check-out, once the room has been inspected and provided that there are no applicable charges for damages, excessive cleaning, missing items, or other expenses resulting from the participant's stay.

Important: The $200 USD non-refundable tour fee and the $100 USD refundable security hold are two separate amounts.

7. CHOOSING NOT TO PARTICIPATE

Choosing not to participate in scheduled wellness appointments — for any reason other than a verified emergency — may result in the loss of sponsored accommodation for that day.

The applicable accommodation charge for non-participation is currently:

$120 USD per day

This amount may be updated by the Program if the applicable accommodation cost changes.

Examples That Do Not Automatically Exempt You:

• Oversleeping
• Forgetting
• Not answering calls
• Intoxication
• Personal conflicts or drama
• Not wanting to attend
• Being late

Exceptions may be considered only for verified emergencies or circumstances approved by Program Administration.

This policy protects the Program structure, provider schedules, and operational costs.

8. DISQUALIFICATION POLICIES

You may be disqualified from returning to a future tour for:

• Choosing not to participate in scheduled wellness activities
• Intoxication that disrupts participation or safety
• Aggressive, disrespectful, or disruptive behavior
• Hotel disturbances or property damage
• Bringing unauthorized guests
• Ignoring staff instructions
• Engaging in unsafe or reckless behavior
• Violating transportation or guest rules
• Interfering with the safety, wellbeing, or harmony of the Program
• Failing to vacate the room upon check-out

If Disqualified:

You may not be allowed to return for a future tour.

9. QUALIFICATION REQUIREMENT BEFORE BOOKING A FLIGHT

To join the Program, you must:

1. Complete an initial phone call with your assigned contact person
2. Submit your required documents: DBL, DD214, and Picture ID
3. Sign this agreement
4. Receive official approval from Program Administration
5. Book your flight after receiving approval

You will not automatically lose your approval if you do not purchase your ticket immediately. However, you must contact the Program again when you are ready to book, because tour availability may change.

If you book a flight without prior approval, the Program may decline access due to limited capacity.

10. HOTEL CHECK-IN / CHECK-OUT

Your luggage may be kept in a secure area at the hotel lobby before check-in and after check-out.

4:00 PM or later

Check-Out

9:00 AM or earlier

If your room has not been vacated by the required check-out time, you are not on the hotel premises, and you cannot be contacted, you authorize the hotel to move your belongings to a designated storage area until you return.

2026 Hotel Schedule

Your stay at Las Flores Hotel & Wellness Center is scheduled from:

Wednesday through Tuesday

Check-out on the final day of the tour is mandatory.

2027 Hotel Schedule

The 2027 tour consists of 6 days.

The starting day of the tour may vary depending on the scheduled tour week. Therefore, there is no standard day of the week on which all 2027 tours begin.

Your exact tour dates and hotel stay dates will be provided in your official tour confirmation.

Participants are required to vacate their room by the designated check-out time on the final day of their confirmed tour.

11. PARTICIPANT ACKNOWLEDGMENT

By signing this agreement, you confirm that:

• You have read and understand all Program rules
• You agree to every section of this agreement
• You understand the financial responsibilities associated with non-compliance
• You understand the transportation and guest rules
• You understand that the Program is not responsible for ER visits or related expenses
• You understand that prohibited behavior may result in disqualification and applicable charges
• You understand the difference between the 2026 and 2027 fees and conditions
• You understand that the 2027 $200 tour fee is non-refundable
• You understand that the 2027 $100 security hold is refundable
• You understand which accommodation policy you are signing
• You understand that 2027 tour dates may vary from week to week
• You agree to follow all Program and hotel expectations during your tour
