Maple Suites - Policies Mandatory Pre-Checking Form The Pre-Checking Form is mandatory and must be completed prior to your tour. If the form has not been completed, you may be contacted through different communication channels as a reminder. Your assigned caller will also contact you to review the information provided in the form. This is important because it allows us to better understand your needs in advance and organise everything accordingly to ensure you have the best possible experience with us. Program Coverage and Guest Policy The Program covers accommodations, meals, transportation, and scheduled activities for the duration of the week for one (1) veteran and one (1) guest. The maximum occupancy per apartment is strictly limited to four (4) individuals. This includes the participating veteran (already counted) and a maximum of two (2) additional guests, regardless of whether they are adults or children. Under no circumstances may the group consist of four (4) adults. If a participant brings a child, the additional adult is mandatory and must remain with the child at all times during wellness sessions and leisure activities. Children are not permitted to attend Program-organized wellness or leisure activities. Children are not permitted to attend wellness activities or leisure activities organized by the Program. For the purposes of this Program, a child is defined as any individual under eighteen (18) years of age. Additional Guest Fees If additional individuals beyond the included guest (e.g., children) are part of the stay and wish to receive breakfast, one meal per day, an additional fee will apply. The following weekly fee are applicable per additional person (this is optional, and you may choose not to include extra participants in meals): Only breakfast is included on weekend days. There will be no additional meal included for those days. Guests & Unauthorized Visitors Guests must be approved Guests must sign this exact agreement Unauthorized visitors are strictly prohibited If a guest violates the rules, they will be asked to leave the premises and the veteran can be removed from The Program The presence of any unauthorized or undeclared guest, including visitors brought into the apartment without prior written approval from the Program, constitutes a violation of Program policies. In the event of an unauthorized guest, a penalty fee of $100 will be deducted from the participant’s security deposit. This penalty applies per incident, regardless of the duration of the unauthorized guest’s presence. Appartment Check-In / Check-Out Your luggage can be kept in a safe space at the lobby before check-in and after check-out. Check-in: 3:00 PM or later Check-out: 8:00 AM or earlier If your apartment is not vacated by 13:00, and you are not on the premises of the property, and you cannot be contacted, you authorize the program to remove your belongings to storage until you return. Your stay at Maple Beach is from Tuesday to Tuesday. Check-out on your last day of tour is mandatory. Payment at check-in The 200$ payment for the week in the apartment will be requested at check-in, as well as the 300$ hold for security deposit. If you refuse to pay and provide the security deposit, you willnot be granted access to the apartment. MANDATORY APPOINTMENT ATTENDANCE You are required to attend ALL scheduled appointments each day. A missed appointment includes: Not attending a scheduled session. Arriving late beyond the allowed grace period. Leaving a session early without authorization MISSED APPOINTMENTS – DAILY PENALTY STRUCTURE If you miss any appointment(s) on a given day, you will be charged: $150 USD for that day. If you miss appointment(s) on a second separate day, you will be charged: an additional $150 USD (total $300 USD for two days). If you miss appointment(s) on more than two (2) separate days, you will be: DISQUALIFIED FROM THE PROGRAM DISQUALIFICATION FROM THE PROGRAM Disqualification will occur if you miss appointments on more than two (2) separate days or fail to comply with attendance requirements. Disqualification means: Loss of access to accommodations (housing). Removal from the tour program and related activities. Continued access to treatments will remain available. No refund of your deposit. Your deposit may be reduced by any applicable charges. You may be responsible for any remaining balance owed beyond the deposit. Policies previously signed Those policies are added to the first policy agreement that you signed before booking your flight. Everything from the first agreement remains accurate and needs to be respected.
