5. GUESTS & UNAUTHORIZED VISITORS

• Guests must be approved by the Program.
• Guests must sign the applicable agreement.
• Unauthorized visitors are strictly prohibited.
• If a guest violates the rules, they may be asked to leave the premises and the veteran may be removed from the Program.

The presence of any unauthorized or undeclared guest, including visitors brought into the apartment without prior written approval from the Program, constitutes a violation of Program policies.

Unauthorized Guest Penalty

In the event of an unauthorized guest, a $100 USD penalty fee will be deducted from the participant's security deposit.

This penalty applies per incident, regardless of the duration of the unauthorized guest's presence.

6. APARTMENT CHECK-IN / CHECK-OUT

Your luggage may be kept in a secure area at the lobby before check-in and after check-out.

Check-In

3:00 PM or later

Check-Out

8:00 AM or earlier

If your apartment is not vacated by 8:00 AM, you are not on the premises of the property, and you cannot be contacted, you authorize the Program to remove your belongings to a designated storage area until you return.

2026 Apartment Schedule

Your stay at Maple Suites is scheduled from:

Tuesday through Tuesday

Check-out on the final day of the tour is mandatory.

2027 Apartment Schedule

The 2027 tour consists of 6 days.

The starting day of the tour may vary depending on the scheduled tour week. Therefore, there is no standard day of the week on which all 2027 tours begin.

Your exact tour dates and accommodation dates will be provided in your official tour confirmation.

Participants are required to vacate their apartment by the designated check-out time on the final day of their confirmed tour.

7. PAYMENT AT CHECK-IN

7.1 Credit Card Authorization

NO AMERICAN EXPRESS — AMEX IS NOT ACCEPTED IN THE DOMINICAN REPUBLIC

By signing this agreement, you authorize the Program to charge the credit/debit card provided for applicable expenses resulting from:

• Non-participation
• Damages
• Excessive cleaning
• Guest misconduct
• Additional expenses caused by your actions

2026 PAYMENT REQUIREMENTS

For 2026 tours:

• $200 USD weekly accommodation fee
• $300 USD refundable security hold

Both amounts are required at check-in.

If you refuse to pay the applicable accommodation fee or provide the required security hold, you will not be granted access to the apartment.

2027 PAYMENT REQUIREMENTS

For 2027 tours:

$350 USD — NON-REFUNDABLE TOUR FEE

This fee is required for participation in the 2027 tour and is non-refundable.

$100 USD — REFUNDABLE SECURITY HOLD

The $100 USD refundable security hold is required at check-in and serves as a security deposit for the apartment.

The hold will be released 72 hours after check-out, once the apartment has been inspected and provided that there are no applicable charges.

⚠️ PLEASE NOTE

The $350 USD non-refundable tour fee and the $100 USD refundable security hold are separate amounts.

Total amount collected at 2027 check-in: $450 USD

Of this amount:

• $350 USD is non-refundable
• $100 USD is a refundable security hold, subject to the conditions of this agreement

If you refuse to pay the applicable fee or provide the required security hold, you will not be granted access to the apartment.

8. MANDATORY APPOINTMENT ATTENDANCE

You are required to attend ALL scheduled appointments each day.

A missed appointment includes:

• Not attending a scheduled session
• Arriving late beyond the allowed grace period
• Leaving a session early without authorization

Participants are responsible for following their daily schedule and arriving on time for all scheduled appointments.

9. MISSED APPOINTMENTS — DAILY PENALTY STRUCTURE

First Separate Day

If you miss any appointment(s) on a given day, you will be charged:

$150 USD for that day.

Second Separate Day

If you miss appointment(s) on a second separate day, you will be charged:

An additional $150 USD

Total: $300 USD for two separate days.

More Than Two Separate Days

If you miss appointments on more than two (2) separate days, you will be:

DISQUALIFIED FROM THE PROGRAM

The applicable charges may be deducted from the participant's security deposit.

If the applicable charges exceed the available security deposit, the participant may be responsible for the remaining balance.

10. DISQUALIFICATION FROM THE PROGRAM

Disqualification will occur if you miss appointments on more than two (2) separate days or fail to comply with the Program's attendance requirements.

Disqualification means:

• Loss of access to Program accommodations
• Removal from the tour program and related activities
• Continued access to treatments may remain available, as determined by the appropriate medical or Program team
• No refund of the applicable security deposit when charges apply
• The security deposit may be reduced by applicable charges
• You may be responsible for any remaining balance owed beyond the security deposit

Disqualification may also occur as a result of serious violations of Program or accommodation policies, including unauthorized guests, disruptive behavior, property damage, or failure to follow staff instructions.

11. OTHER PROGRAM POLICIES

This agreement is an additional policy agreement that supplements the first Program policy agreement signed before booking your flight.

All policies and requirements included in the first agreement remain valid and must continue to be respected.

By signing this agreement, you acknowledge that:

• This agreement does not replace the first policy agreement.
• Both agreements apply to your participation in the Program.
• You are responsible for following the requirements outlined in both agreements.
• Any conflict or question regarding Program requirements should be directed to Program Administration before taking action.

12. FINANCIAL RESPONSIBILITY

Participants are responsible for applicable charges resulting from:

• Missed appointments
• Unauthorized guests
• Damages to the apartment or property
• Excessive cleaning
• Guest misconduct
• Failure to follow Program or accommodation policies
• Other expenses caused by the participant or their guests

Applicable charges may be deducted from the participant's security deposit.

If applicable charges exceed the available security deposit, the participant may be responsible for the remaining balance.

13. EMERGENCY ROOM (ER) VISITS

The Program is not responsible for ER visits or related expenses.

If you choose or need to go to the ER:

• You are responsible for all medical or hospital bills
• You are responsible for transportation
• The Program does not reimburse ER expenses
• The Program is not responsible for delays or consequences resulting from an ER visit

This Program does not include emergency medical coverage.

14. PARTICIPANT ACKNOWLEDGMENT

By signing this agreement, you confirm that:

• You have read and understand all Program rules contained in this agreement and the first Program agreement.
• You agree to every section in this agreement and the first Program agreement.
• You understand the applicable accommodation fees for your tour year.
• You understand that the 2026 Maple Suites accommodation fee is $200 USD.
• You understand that the 2026 security hold is $300 USD and is refundable, subject to applicable charges.
• You understand that the 2027 tour fee is $350 USD and is non-refundable.
• You understand that the 2027 security hold is $100 USD and is refundable, subject to applicable charges.
• You understand that the 2027 tour consists of 6 days.
• You understand that 2027 tour dates may vary from week to week.
• You understand the transportation and guest rules.
• You understand the maximum occupancy requirements.
• You understand the additional guest fees.
• You understand the $100 USD penalty for unauthorized guests.
• You understand the mandatory appointment attendance requirements.
• You understand the financial penalties associated with missed appointments.
• You understand that missing appointments on more than two separate days may result in disqualification.
• You understand that the Program is not responsible for ER visits or related expenses.
• You understand that prohibited behavior may result in disqualification and applicable charges.
• You understand all financial penalties and consequences described in this agreement.
• You agree to comply with all attendance requirements.
• You agree to follow all Program, accommodation, and staff instructions during the tour.
